Graeme has extensive leadership experience in the corporate and welfare sector in youth, family and disability services.  He is a values-based leader and is passionate about making a difference in helping people achieve their goals.

Jay Kodithuwakku

Chief Financial Officer (Dip. IT, BCom (Accounting), CPA)

Jay is an accomplished finance executive with over 15 years’ experience in the not-for-profit sector. He leads the organisation’s financial strategy, providing strategic advice to the Committee of Management and Executive team to support long-term sustainability. Known for his collaborative leadership style, he works closely with leaders across the organisation to align financial performance with strategic and community impact goals.

Rosemary Nicholls

Support Coordination Manager

Rosemary is an experienced social worker. She has spent 25 years working with families and children in the areas of permanent care/foster care, volunteer management and disability..

Roz Talib

Plan Manager

Roz is our Plan Manager Team Lead with past experience in Accounts and Auditing. As an NDIS Plan manager for seven years, she has developed her skills and expertise in NDIS plan management, guiding our PM Team and other staff of Extended Families.

Nicola is a qualified social worker and holds a bachelor of Social Science and Honours in Social Work. Nicola is passionate about empathy, dignity and respect for all, and enjoys working in partnership with young people and their families to support them to live an inclusive and meaningful life.

As an experienced and compassionate NDIS Support Coordinator, Quyen is dedicated to empowering individuals with disabilities to lead independent, fulfilling lives. Fluent in both English and Vietnamese, Quyen brings a unique ability to support diverse communities, ensuring that participants from various cultural backgrounds feel heard, understood, and supported. With a deep understanding of the NDIS, Quyen works closely with participants to navigate the complexities of the system, ensuring they access the services and supports they need to achieve their goals. Drawing from a background in in social work with 19 years’ experience in the welfare and community service industry, Quyen excels at developing personalised support plans that promote choice, control, and community integration.

Chi comes from Nursing background and has more than 5 years of experience in disability sector. As a Support Coordinator, her approach focuses on collaboration and individualised support that thrive in empowering participants to build up independence to achieve their goals, with the help of right support system

Hung is passionate about working with those living with a disability and their families.  He has a background in Disability Employment, and drug and alcohol support for clients from a range of nationalities.

Megan Sapiano

Support Coordinator

Megan is a Support Coordinator with over 15 years of experience in disability in a variety of roles. Megan’s motivation is to empower individuals to reach their fullest potential and she is committed to providing a person-centred approach to service delivery.

Vi has a background in international/community development and has worked in various community settings. She aims to support her participants achieve their personally set goals through a person led and strength base approach. Vi is passionate about supporting young people and the CALD community.

Doris is a qualified social worker with over 10 years of industrial experience in areas of disability and family services.  Doris is committed to disability inclusion and takes a person-centred and strength-based approach to work with participants and their families to achieve the best outcomes. Doris is fluent in both English and Mandarin.

Quyen is a qualified social worker with over 8 years experience in mental health and disabilities. She is passionate about supporting people to achieve their goals through person-centred practice and strength-based approach.

Support Coordinator

Shuhan holds a bachelor's degree in sociology and a master's degree in social work. The education background has allowed her to see people through both structured, societal lenses and deeply human, individual stories. She feels privileged to bring this knowledge into her daily practice, building authentic connections with those she supports and walking alongside them through life’s highs and lows. She approaches each relationship with empathy, respect, and a commitment to genuine care.

Alison Lambden

Support Coordinator

Alison has over 10 years’ experience in the disability sector and was a former employee of the National Disability Insurance Agency.  Most recently Alison has worked as a Support Coordinator and Specialist Support Coordinator since 2017 and specialises in supporting people in transitional phases of their lives.  Alison brings to the role her skills and expertise in navigating Government systems and support services in the Community and is passionate about supporting people to build strengths, develop skills and to assist people with a disability to create meaningful community connections and realise their full potential.

Amy Kam

Support Coordinator

Amy has a background in aged care, who transitioned to working with clients with disabilities two years ago, She specializes in supporting people of all ages, from children to older adults, with a diverse range of disabilities. She actively engages with and supports the families of their clients. She speaks English, Cantonese and Mandarin.

Lauren Martin

Support Coordinator

Lauren is a dedicated NDIS Support Coordinator with over 6 years of experience in the disability sector. Lauren has a Bachelor of Health Sciences with majors in disability, families, and health promotion studies, and is also currently completing her Master of Social Work. Lauren is passionate about advocacy and empowering individuals with disabilities to achieve their goals through a client centred and compassionate approach.

Wynnie Ma

Support Coordinator

Wynnie speaks English, Cantonese, and Mandarin, and has a background in nursing with experience across the disability and aged care sectors. She is passionate about supporting participants and their families to work towards their goals and build meaningful connections in their community.

As a dedicated team member of Plan Management, Long brings a wealth of expertise and a personal touch to his work. He is particularly passionate about fostering close relationships and providing exceptional support to our Vietnamese clients, ensuring they receive the best possible service

Rebalance Coordinator

Ahysa brings with her more than 20 years experience in service delivery, the last 4 being specifically in the disability sector. She is passionate about providing people-focused assistance and meeting participants’ needs and expectations while keeping to program/plan budgets and constraints.

As a member of Plan management team, Peany is enthusiastic in building companionship with families and participants from different backgrounds as well as supporting them with managing financial aspect of their NDIS plans.

Jess is a qualified social worker with 20 years experience in the community sector, with special interest in program design, content development and evaluation. Jess is passionate about program co-design with service-users, and embedding ongoing review and consultation processes to ensure desired outcomes of service-users are met.

With a background in teaching, disability and social development, Nghi believes everyone has the power to create change.  Her passion is working alongside youth with special needs and their families to support them to bring about positive change for themselves.

Samantha-Jane Potter

Social Groups and Volunteer Coordinator

Samantha-Jane Potter, a compassionate Social Groups and Volunteer Coordinator, creates meaningful events for disabled children, youth, and their families. With experience in care management and event coordination, she holds a Postgraduate degree in Arts and Community Engagement, an Art’s degree in drama and music, and a Diploma in Leisure and Health. Samantha-Jane fosters inclusive spaces and promotes well-being through thoughtful programming and collaboration

With background in teaching children with special needs, Mai is employed as a facilitator, organizing events for children and young adults with disabilities. She believes that giving a little encouragement can make a significant difference in a person’s life.

Emily has been a MyTime facilitator for since 2021, for a group of Chinese parents. She has an autistic adult son, which helps her understand the stress of being a caregiver. Drawing on her loved experience, Emily is passionate about supporting new migrant families and their children in navigating supports systems.

Cindy has a background in Behavioural Science and Human Resources Management. She is passionate about translating the HR knowledge that she has into practice and contributing to the organisation’s wider HR strategy.

Experienced finance and accounting generalist in public, retail, shared services and audit sectors, Nura currently oversees financial accounting operations in the organisation.

Olivia has a wealth of experience promoting community inclusion and participation through sport and recreation. With a strong background across the sport, community, and disability sectors, she has taken on a variety of impactful roles at Extended Families. Today, Olivia enjoys a dynamic position that combines her passion for connection and communication - managing social media and providing administrative support.
